Download form SF100 (Sure Start Maternity Grant claim form) from GOV.UK.
Have your midwife, doctor, or health visitor sign the health professional statement on the form.
Post the completed form to Freepost DWP SSMG or submit it through your local Jobcentre Plus.
The Sure Start Maternity Grant is tax-free and does not count as income for local Council Tax Reduction or Housing Benefit calculations.
Financial support for pregnant individuals who cannot get Statutory Maternity Pay from an employer (e.g., self-employed, short-tenure, or low-earning workers).
