    Apply online for 'New Style ESA' via GOV.UK.

    Provide a 'fit note' (med 3) from your doctor or healthcare professional.

  3. 3

    Complete the 'Capability for Work' questionnaire (ESA50) and attend a Work Capability Assessment if required.
